Intensive Case Management for the Most Vulnerable 

This program assists populations that face additional barriers to self-sufficiency. Many clients have survived traumatic experiences that have negatively impacted their mental and emotional health. Other clients suffer from physical disabilities and illness. Our case managers work hard to provide critical resources to these clients, ensuring that they receive quality healthcare and community support. In essence, we work toward removing social, economic, and health-related obstacles to allow clients to integrate into American society.
